In Plain English

Your organization must define exactly which events are logged across all systems -- logons, access attempts, privilege usage, configuration changes, and data access at minimum. Each log record must capture what happened, when, where, who, and the outcome. The logging strategy must be documented, justified, and reviewed periodically.
